Dear Sir, With reference to the above subject and with reference to the Health insurance certificate policy no. 10406748, I had submitted the application to State Bank of India for taking the health insurance policy for my mother but they had made the policy for me instead of my mother and they wrote my mother`s name as a wife in the health insurance certificate. which is absolutely wrong.When my mother told to the Bank Manager for the above and trying to give the application for making the correction. But she didn`t receive and denied her mistake and said to my mother that the policy was made as per the submitted form. When the form was asked she denied again. I am attaching the copy of Health Insurance Policy for your review. Thanks
